Typically, the X server sends expose events to the windows, including the window for this drawing area. The widget then forwards the event to you in the form of an XmNexposeCallback. The callback procedure in your application then re-does the drawing, as needed.

If you want to redraw without first receiving an expose event, you can simply clear the area and then call your internal routines for redrawing.
